Athletic Forms

Midview Local Schools, including its Athletics Department, continues to exclusively use online forms in 2026-27.  

The only paperwork needed to turn in to the Athletics Department will be Page 1-5 of the OHSAA mandated participation physical form. These shoud be uploaded as a single PDF into FinalForms for approval.

All other forms—Emergency Medical, Code of Conduct, Insurance Waiver, Concussion and Sudden Cardiac Arrest—will be filled using the FinalForms program, which is connected directly to PowerSchool. Parents/Guardians of student-athletes will need to complete these online forms prior to any tryout or practice.  Paper versions of these forms will no longer be accepted.

The forms have already been transferred from their 2025-26 versions in the FinalForms Portal. We are requesting that you take a moment to visit the FinalForms portal HERE and update your forms.
